Once you submit your application, we will automatically email that teacher a link to complete the recommendation form. The teacher must complete and submit the form themselves through this link. We cannot accept recommendations sent by parents or from non-teacher email addresses.
All of these items should be scanned as pdf's and uploaded electronically during the application process. Jpeg is not an acceptable file type as it does not print legibly.

At the time of application, all new students seeking admission to Assumption will be evaluated on the basis of standardized test scores, report cards, teacher recommendations, admissions testing, and interviews. Students applying for admissions are required to upload the above-listed required documents. These will be reviewed to determine whether the program at Assumption Catholic School would meet the educational needs of the student.
As openings become available, the following priorities are used to accept students to Assumption Catholic School:

After applications are reviewed, some candidates may be invited to participate in admissions testing and interviews as part of the application process. These steps allow us to learn more about each applicant and support our commitment to ensuring a strong fit for both the student and the Assumption Catholic School community. Participation or non-participation in any phase of the admissions process should not be interpreted as an admissions decision; all final decisions are communicated via email.
ADMISSION DECISION/ENROLL

Students admitted are required to complete the online enrollment and submit the enrollment fee by the date indicated in the Enrollment Offer email. Failure to meet this deadline will move the applicant to the waitlist.
How many openings will you have in each grade level and what are your admission priorites?
A.We open applications to grade levels based on current enrollment.
Our school campus is kindergarten through eighth grade. While we cannot guarantee the exact number of openings until we have completed our current student re-enrollments in January, we warmly welcome new students at various grade levels every year. If you are considering Assumption Catholic School for your child, we encourage you to apply.
As openings become available, the following priorities are used to accept applicants to Assumption Catholic School:
1. Registered and active Assumption Catholic Church Parishioner
2. Sibling of current registered and active Catholic Assumption student
3. Registered and active members of other Catholic parishes
4. Child of parent or sibling alumni
5. Non-Catholic students
Does participation in any part of the admissions process indicate an admissions decision?
A.No. Participation (or non-participation) in any phase of the admissions process should not be interpreted as an admissions decision; all final decisions are communicated in March.
My child’s current school requires re-enrollment before Assumption releases admissions decisions. What should we do?
A.Families are encouraged to make the best decision for their child based on the information available at the time. Since Assumption Catholic School releases all final admissions decisions in March, we recommend that families maintain their current school enrollment until an official decision is received. This helps avoid the risk of losing a secured placement while the admissions process is still underway.
Does Assumption Catholic School offer tours and shadow days?
A.Assumption Catholic School offers individual tours that both parents and prospective students can attend. These can be scheduled on our website at the VISIT ACS tab. Due to the number of applications received, we do not offer student shadow days.
If my student currently attends another Diocese of St. Augustine school can they transfer to Assumption Catholic School?
A.No, all students who wish to enroll at Assumption Catholic School must go through the entire admissions process and submit an application by the published due date.
Do you accept 8th grade students?
A.When space allows, 8th grade transfer applicants may be considered from non-local Catholic schools.
What is your student-teacher ratio?
A.Our class sizes are purposefully kept small to ensure individualized attention and support student success. We limit class sizes to 20 students in K–4, where each grade level also has a teacher aide, and 25 students in grades 5–8.
Does your school offer Financial Aid?
A.Yes, we accept Step-Up, Family Empowerment Scholarships (FES-EO, FES-UA), Florida Tax Credit Scholarship (FTC), FACTS Grant & Aid applications, and the Parishioner Rate application. More information available under the FINANCIAL TUITION & SCHOLARSHIPS tab.
What after school opportunities are available to students?
A.We offer school aftercare until 6:00 p.m. for an hourly rate. More information available under CAMPUS LIFE- AFTERCARE.
We offer many Junior Varsity and Varsity sports options with tryouts starting in 4th grade. We also offer a recreational Basketball League (ABL) for PreK4-8th grades. More information on our sports programs available under CAMPUS LIFE- ATHLETICS.
After school club options vary but range from Robotics to Dance and parents are given the opportunity to enroll in these programs once school starts.
ENROLLMENT/ RE-ENROLLMENT
OFFICE RECORDS
Parents/Guardians must update any change of address, home telephone numbers, cell phone numbers, business phone numbers, e-mail addresses and/or phone numbers of emergency contacts in their Family Portal account. This will guarantee that office records are accurate, complete, and up to date.
STUDENT RECORDS
Assumption Catholic School adheres to the Buckley Amendment (Family Education Rights and Privacy) regarding access to student records. Records of students transferring to other schools will only be sent through the US Mail or scanned and emailed directly to the school registrar. No records will be given to parents to transport to the new school.
Current enrolled or previous Assumption students or their parents requesting records/transcripts/recommendations must allow five school days in order to fulfill the request. Records will be sent via the U.S. Mail or sent electronically. Special handling will require that all postal fees be paid by the student's parent/guardian.
All student evaluations will only be sent directly to the office requiring the evaluation. No copies of the evaluation will be given to parents.
No records will be sent to transferring schools of students whose financial commitment is in arrears.
IMMUNIZATIONS
Florida Statute 232.0332, which became effective on August 1, 1982, requires that prior to admittance to or attendance in a public or non-public school, grades kindergarten through 12, or a preschool, each child present must have on file with the school a certificate of immunization (DH 680) for the prevention of those communicable diseases for which immunizations are required by the Department of Health.
FL Department of Health regulations require DH 680 Form, Part C, to be signed by hand or by electronic signature in Florida SHOTS by a physician licensed under Chapter 458 MEDICAL PRACTICE or Chapter 459 OSTEOPATHIC MEDICINE, Florida Statutes.
OPEN ADMISSION POLICY
Assumption Catholic School has an open admission policy. No person, on the grounds of race, color, gender, disability, or national origin, is excluded or otherwise subjected to discrimination in the receiving of services. Nor does the school discriminate in hiring, promotion, discharge, pay, fringe benefits, job training, classification, referral, and other aspects of employment on the basis of race, color, disability, age, gender, or national origin.
TITLE IX
Assumption Catholic School adheres to the tenets of Title IX: “No person in the United States shall, on the basis of sex, be excluded from participating in, be denied the benefits of, or be subjected to discrimination under any education program or activity receiving federal financial assistance.”
IDEA
A parentally-placed private school student with a disability does not have an individual right to receive some or all of the special education and related services that the student would receive if enrolled in a public school under the Individuals with Disabilities Education Act (IDEA).
